If you are a music teacher with one or more students in MYA, use this form to request one (1) complimentary ticket for each concert in which your student(s) perform.

Additionally, each MYA student is entitled to one (1) complimentary ticket to any concert in which he/she doesn’t perform.

Also, if you have made a donation to MYA of $1,000 or above, you are entitled to four (4) tickets per concert.
